To request for increase in contribution(s).

● Service Request - Amendments to Contingent Investment Adviser, Beneficiaries or Letter of Wishes Form

Service Request - Amendments to Contingent Investment Adviser, Beneficiaries or Letter of Wishes Form

● Service Request - Asset Switching and Redirection Form

To request for asset switch or change in asset allocation.

● Service Request - Change of Personal Information Form

To request for change of personal details (contact details, personal particular, signature and PEP Status) and including personal data use statement and servicing adviser).

To request for assignment of insurance policy into the Trust Plan.

To request for nomination of insurance policy into the Trust Plan.

● Service Request - Plan Alteration Form (For existing Plan use)

To request for change of payment method, contribution frequency/amount, reinstatement, settlement of outstanding contribution, etc.

● Service_Request - Provident_Fund_Nomination_Request_Form

To request for nomination of Provident Fund into the Trust Plan.

● Service Request - Withdrawal / Encashment Request Form

To request for withdrawal / encashment from Trust Plan.

● Guideline - Payment Method

Guide on how payment of contribution(s) can be made via Giro , Cheque, Credit Card , Telegraphic Transfer and PayNow.

● Guideline - Supporting Documents for Large Amount Contribution

Guide on the necessary documents needed for application involving large contribution amount. (Large Amount is defined as regular contribution amount > S$50,000 per annum or one lump sum contribution >S$500,000).

● Guideline - Supporting Documents Required Under Specified Events

Guide on the necessary documents to be submitted in the events of deceased or incapacitation the settlor(s) and/or beneficiaries.

● Others - Business Reply Envelope

To be used as a prepaid envelope for documents to be mailed to Metis SG.

● Common Reporting Standard - CRS Self Certification Form (Individual)

For individual to submit any changes in tax and residency status to Metis SG.
